As part of the Serious Incident Reponses Scheme (SIRS), providers are responsible for submitting notifications about serious incidents via the SIRS tile on the My Aged Care Provider Portal.
The Commission has created the below example responses to help providers understand the level of detail that is required to ensure the Commission can appropriately assess the incident and their response to it.
Any similarities to an actual reportable incident or individual is purely coincidental.
