Submitting a SIRS notification

    From 1 December 2022, the Serious Incident Response Scheme will be extended from residential aged care and it will also apply to home care and flexible care delivered in a home or community setting.

    As part of the Serious Incident Response Scheme (SIRS), residential aged care providers are responsible for submitting notifications about serious incidents via the SIRS tile on the My Aged Care Service Provider Portal.

    The Commission has created the below example responses to help providers understand the level of detail that is required to ensure the Commission can appropriately assess the incident and their response to it.

    Any similarities to an actual reportable incident or individual is purely coincidental.
