You must notify us of any change of circumstances or event that affects, or may affect, your suitability to be a registered provider.
If you don’t provide enough detail about the reasons for each change or event, and how they affect suitability, we can’t:
- confirm if you’ve fully considered your obligations
- be satisfied that the change or event doesn’t put the older people you care for at risk. This will cause processing delays.
You need to include documentation to support your answers.
What is suitability?
A provider’s suitability to be a registered provider relates to:
- your compliance with relevant federal, state or territory laws, including any past non-compliance
- if there is, or has been, a banning order or an NDIS banning order against your organisation
- if your registration in aged care or the NDIS has been revoked (cancelled not on your own request) or suspended
- your previous experience and performance in delivering funded aged care services
- your legal and business structure.
Some organisation changes can affect your ability to meet your registered provider obligations. They can also change your responsible person, which you must tell us about.
When you notify us about a change, we’ll look at whether there are any risks to you meeting your legal obligations in the:
- new Aged Care Act 2024 (new Aged Care Act)
- Aged Care Rules 2025.
The new Aged Care Act requires you to make a written statement in an approved form that describes:
- what effect the change has on your suitability to be a registered provider
- what you’re doing to make sure that you’re meeting your obligations.
When you complete the form, include as much information as possible to explain what has changed and why. If you don’t provide enough detail, we may ask you for more information. This will delay the process.
All providers should regularly check their legal obligations. This can help you identify if changes your organisation makes could affect your ability to meet your obligations or your suitability to be a provider.
Notifying us of a change in provider suitability
To notify us of a change in your suitability to be a registered provider, you must complete the:
- Change in circumstance notification form
- Change to provider suitability to be a registered provider form.

This form is about changes that could affect your suitability to be a registered provider, and to continue to provide funded aged care services.
You need to:
- fill in each field on the form that relates to the change
- include enough detail so we can work out if you’ve fully considered your obligations, particularly about the reason for the change and how it affects your suitability.
