Significant change in the scale of the provider
You must notify us about:
- a large increase or decrease in the number of your aged care workers
- a change to the local government area that you provide funded aged care services in
- any other significant change in scale.
Notifying us of a significant change in scale
To notify us of a significant change in your scale, you must complete the:
- Change in circumstance notification baseform
- Significant change in the scale of the provider subform.

When completing the forms use the Change in Circumstance Guidance. It outlines what evidence and supporting documents you’ll need to attach.
Your notification must include:
- details of the change
- the services that will be (or are likely to be) affected by the change
- any other expected effects of this change
- the reason for the change
- any actions that you’ve taken, or plan to take, to manage the effect on the services.
