Serious risk decision

Following an assessment contact conducted on 4 December 2018, a decision was made that UnitingCare Wesley Port Adelaide Ltd, trading as UnitingSA failed to meet one expected outcome in the Accreditation Standards in relation to Hawksbury Gardens Aged Care Facility.

On 15 January 2019 a decision was made by a delegate of the Commissioner that the failure to meet one of these expected outcomes has placed the safety, health or wellbeing of care recipients of the service at serious risk. The Department of Health has been notified of the risk.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

The Commission will continue to monitor the performance of the service including through unannounced visits.
