My Aged Care provider portal planned outage

The My Aged Care Service Provider portal is currently undergoing schedule maintenance from

4:15 pm Saturday 16 May 2026 (AEDT) to 11:55 pm Saturday 16 May 2026 (AEDT).

Registered providers who need to submit a Serious Incident Response Scheme (SIRS) notice during the outage to meet their reporting deadline should download and complete the SIRS Smart Form using a PDF reader application, and submit the form to Form.SIRS@agedcarequality.gov.au.

The previous SIRS manual form is no longer accepted and registered provides must complete the SIRS smart form.
