The My Aged Care Service Provider portal is undergoing schedule maintenance from:
9:00 am Sunday 12 July 2026 (AEDT) to 6:00 pm Sunday 12 July 2026 (AEDT)
Registered providers who need to submit a Serious Incident Response Scheme (SIRS) notice during the outage to meet their reporting deadline should download and complete the SIRS Smart Form using a PDF reader application, and submit the form to FORMS.SIRS@agedcarequality.gov.au.
The previous SIRS manual form is no longer accepted and registered provides must complete the SIRS smart form.
