Section 1: Applicant type

These questions relate to sections 109(1) (a) and (b)(vi) of the Aged Care Act 2024

Questions 1.1 to 1.5 

Explanatory Note

You must be one of the entity types listed in the form to apply for registration as a registered provider.

You must have an ABN to be registered as a registered provider. If you are applying as the trustee of a trust, use the ABN related to the trust. Do not use another ABN that you hold in a different capacity.

Change in trusteeship

If you become a registered provider and plan on a change to the trusteeship in the future, please make sure that the:

  • outgoing trustee (you); and

  • incoming trustee (another entity)

advise us before that change in trusteeship occurs. A failure to do so may impact your registration and the registration process for the incoming trustee.

Evidence

If you have selected ‘Yes’ to question 1.2, you must provide: 

  • the name of the trust

  • the ABN related to the trust (i.e., the ABN you have in your capacity as trustee of the trust)

  • a copy of the current trust deed.

Section 2: Business registration details

These questions relate to sections 109(1)(a) and (b)(vi) of the Aged Care Act 2024

Questions 2.1 to 2.15

Your response to these questions helps us to understand your legal structure and how you operate your business. This helps us to assess your suitability to deliver funded aged care services.

If you have provided the ABN related to a trust at question 1.4, repeat that ABN at question 2.1.
