Question 1.1 to 6.1

Explanatory Note

Provide information about each of your responsible persons. 

A responsible person is someone that is:

  • a registered nurse with responsibility for overall management of the nursing services delivered by you or at your approved residential care home

  • responsible for the day-to-day operations of your service delivery branch or at your approved residential care home. 

A service delivery branch is a place of business through which you deliver funded aged care services to an older person in the following service groups (see also question 1.2 in Part E: Service delivery):

  • home support

  • assistive technology 

  • home modifications. 

A service delivery branch can include locations that support care delivery, even if care is not provided directly at those sites (e.g., a central office).

If you are not a government entity, a responsible person is also someone who:

  • is responsible for executive decisions which includes each member of your governing body

  • has authority, responsibility for, or significant influence over the planning, directing or controlling of your activities. 

The person identified as “Responsible person 1” on the form will become the key contact for this application. If we need to contact you during the application process, we will communicate with the key contact. If a consultant has assisted you with completing this application, they cannot be the key contact unless they also meet the definition of a responsible person.
